The author presents his experience in working with a large number of persons diagnosed with psychotic and borderline disorders in long-term relational psychosis psychotherapy, particularly the challenges and difficulties involved, as well as the opportunities for therapeutic action and efficacy.  Relational psychosis psychotherapy is described, as is relevant research in social neuroscience, including research on the dynamic social genome and epigenome.  An integration is attempted along the lines of therapeutic efficacy.
